An East County Health Outreach and Education ProgramIn January 2008, Center for Human Development joined in a new partnership with Contra Costa Health Services to address health disparities among underserved populations.
Based at the Bay Point Family Health Center, the Promotoras serve the Spanish-speaking community of East and Central County with support for accessing health care and health education workshops. The classes cover such topics as managing chronic disease, nutrition, and HIV-AIDS. The African American Health Conductors are also based at the Bay Point Family Health Center and help African American residents of Bay Point and Pittsburg navigate the county’s health care system. They also facilitate an educational support group known as “Soul to Soul” and conduct outreach on HIV education and prevention.
